Phase 1 Property Information Form

Submission of the information requested on this form, will assist staff with the Phase 1 inspections under the Town's Sewage System Maintenance Inspection Policy/Program.  Owner's  contact information is required to enable staff to follow up with property owners following on-site inspections.  The information collected will also help inspectors identify what is on the property in advance of the inspection.    Thank you for taking the time to assist.

Property Sketch

Please attach a sketch of your property clearly showing road and lake frontage, all buildings and structures, septic tank, septic field and well locations.  The sketch does not have to be drawn to scale but should include distances between property lines/buildings and specific distances between well and septic.  For more information visit the Ontario Onsite Wastewater Association Resources.
